Certification Request Form
You must submit a Certification Request Form at the beginning of each semester and whenever you add, drop, or swap a class. Failure to submit this form will result in your benefits not being processed.
Parent Institution Letter
The Parent Institution Letter (PIL) Request Form is required for military-connected students who are enrolled at the University of Houston-Clear Lake (UHCL) as their parent institution (degree-granting school) and plan to take courses at another college or university (guest institution). This form allows UHCL to verify that the requested courses taken at the guest institution apply toward the student's degree program and authorize the guest institution to certify those courses with the U.S. Department of Veterans Affairs (VA) for education benefits.
Students must submit this completed form each term they wish to take courses at another institution. A Parent Institution Letter will only be issued for courses that are part of the student’s approved degree plan.

Hazlewood Act
Great news, Hazlewood recipients! Less paperwork starting Fall 2024. Submit the Exemption Application once annually (This fall covers spring and summer semesters too!). Semester updates are a breeze - just use the Hazlewood Certification Request Form every beginning of the semester and every time you add, drop, swap or withdraw.
